How Do I Add A Taxonomy Code To My Claim Form?

This article will demonstrate the areas where a Taxonomy code can be displayed on a HCFA 1500 form.

If you have a Payer requirement to display a Taxonomy code on your HCFA claims form, this will normally display in either HCFA Box 24j or Box 33b. 

This setting can be managed in your global insurance company settings > HCFA 1500 tab.

HCFA Box 24j

You must select the Qualifier for Taxonomy and enter the code:

HCFA Box 33b

You should choose the ID Qualifier for ZZ- Provider Taxonomy from the drop-down menu and enter the code in Box 33b. Afterwards, make sure to check the box that says "Use Box 33b" as shown in the image below.

This requirement is normally payer specific and you should verify with individual payers as to the exact requirements prior to customizing these settings.
