This articles covers the steps involved to edit Box 11 on the HCFA 1500 claim form.
To edit Box 11 on the HCFA 1500 claim form, go to Settings > Insurance > Add/Edit Insurance Company. If the insurance company already exists, select the company to edit from the menu. You have the ability to edit Box 11 directly from the HCFA 1500 tab as shown below.
If you select the checkbox adjacent to Box 11, it will pull the Group Number entered on the patient's Primary insurance page and disable the text field.
To include information that will appear on all claims pertaining to a specific Insurance company, leave the checkbox unchecked and enter the desired information in the Box 11 text field. The system will apply this data on all claims for that specific payer, instead of pulling the Group/Policy number from patient profiles.
