Completing online intake forms during patient self check-in

Patients who have not completed their intake paperwork prior to arriving for their appointment at your office, are able to complete it at check-in.

You patients have the ability to complete their digital intake paperwork from your waiting room at check-in. Multiple devices can be designated in your office for extra convenience. For example, one device can be used strictly for patient self-check-in and a second device can be used for self check-in and for completing online intake forms.
